Symptoms of Paracetamol Toxicity

Abdominal pain, vomiting, hypoglycemia, jaundice, hepatomegaly (increase liver size).

Diagnostic test

CBC or FBC, Urea and electrolyte test, LFT, prothrombin index and sugar test should make if ingestion time is more than 8 hours.

Immediate Treatment

Administered 50% dextrose 50mL IV (NOTE: Review blood reports if patient is hypoglycemic)

Plot the serum Paracetamol level on the Paracetamol nomogram for all patients presenting between 4 and 24 h after an acute if non single ingestion of Paracetamol (if nomogram is available) . The treatment nomogram has been simplified from previously, with a single treatment line now for all patients following Paracetamol ingestion. A raised PTI (INR) or alanine aminotransferase (ALT) levels >1000 IU/L define significant hepatotoxicity.

NAC Injection Administration

N-Acetyl Cysteine injection should administer to patient having ingestion within 8 hours.

Initiate administration of NAC immediately without waiting for results of diagnostic and lab values. Stop treatment if ALT or INR level drops to normal. It should also note that serum Paracetamol levels are difficult to interpret in cases of staggered ingestion. Monitor the PTI (INR) and ALT regularly instead and seek specialist toxicologist advice. For administration of NAC, use adult infusion protocol for NAC below.

(1) Accurately calculate dose of NAC. Read from the drug-insert infusion dosage guide the volume in millilitres of NAC 200 mg/ mL to be added to the 5% dextrose, according to the patient’s weight. Weight specification are:

(a) 150 mg/kg in 5% dextrose 200 mL IV over 15 min.

(b) 50 mg/kg in 5% dextrose 500 mL IV over 4 h.

(c) 100 mg/kg in 5% dextrose 1000 mL IV over 16 h.

(2) Stop the infusion :

(3) Give Promethazine 12.5–25 mg I.V and hydrocortisone 200 mg IV.

(4) Once symptoms have settled, re-commence the initial infusion at a slower rate (e.g. 150 mg/kg over 1 h).

Parkinson’s disease is a neurological condition that affects the sufferer’s mobility and speech. It affects the central nervous system (Brain), leading to a person’s inability to control muscles throughout the body. It is a disease whose treatment does not intend to cure it but aims are controlling its manifestation. There are various ways of treatment of Parkinsonism. It may involve the use of drugs or the performance of surgery. Doctors strongly recommend exercise to keep muscles strong. It progresses gradually or severely from the early stages with mild symptoms and severe symptoms in later stages.


Objective of Treatment

The disease is a result of lowered formation and action of a brain chemical (Neurotransmitter) dopamine which is produced by the dopaminergic neurons found in the brain. This knowledge leads to the formation of drugs that will either increase the formation or mimic the actions of dopamine in the brain. This is important because an increase levels of dopamine results in of the affects of the disease. All the drugs which are used to treat Parkinson’s diseases contain dopamine as an active ingredient, regardless how it works.
Early Treatment of Parkinson’s Disease
Among the earliest forms of treatments are the anticholinergics. These were used even before the introduction of Levodopa, which is now the most widely used drug. The anticholinergics drugs administration had very little benefits as compared to its harmful side effects. These side effects include dryness of mouth, urine retention (which especially occurs among men), and very severe constipation and nausea. There are some other side effects that are more serious and the major reason for the minimal use of these drugs. These include confusion, memory loss, and hallucination.
Now the prescriptions of these drugs have rarely seen. Since some anti-depressants and antihistamines tend to have almost the same effect as the anticholinergics, so doctors prescribe them to older people who are suffering from this terrible disease. Once an individual experiences any of the serious severe symptoms, stop these drugs immediately.


Levodopa

Levodopa is a drug that was introduced in 1967 and is currently the most commonly used drug. It is used to help to restore the control over ones muscles. Levodopa is referred to as the gold standard of Parkinson’s treatment because it is usually used as the first line of defense. The reason a sufferer of Parkinson’s can’t just take Dopamine itself is because Dopamine cannot breakthrough the blood-brain barrier (A check point, composed of blood capillaries that allow specific substance entrance in brain). Levodopa can get through this barrier and is converted to Dopamine once it reaches the brain. It therefore increases the level of Dopamine and counters the effects of the disease.
Levodopa however has very serious side effects which include nausea and dyskinesias (involuntary movement). This occurs because only a small percentage of Levodopa actually reaches the brain that transformed into Dopamine. The rest of it is transformed into dopamine in the body. It is therefore becomes necessary to use Carbidopa, which delays the metabolism (Breakdown) of Levodopa until it reaches the dopaminergic neurons in the brain. Levodopa-Carbidopa combination helps to reduce the side effects caused by single use of Levodopa.
The other problem posed by the use of this drug is that its dosage needs to be increased over time. This makes it unreliable because its effects are not constant, patient may suffers from violent attacks while still under medication. The various side effects and unreliability of Levodopa leads to the necessity to combine it with various other drugs to make it more effective. Some of these drugs can be used on their own while others actually require to be used with the Levodopa-Carbidopa combination.

Researchers at the J. Craig Venter Institute (JCVI) published results on Thursday 20,May 2010 describing the successful construction of the first self-replicating, synthetic bacterial cell. The synthetic cell is called Mycoplasma mycoides and is the proof of principle that genomes can be designed in the computer, chemically made in the laboratory and transplanted into a recipient cell to produce a new self-replicating cell controlled only by the synthetic genome.

Daniel Gibson and his colleagues at the J. Craig Venter Institute in Rockville, Maryland, synthesized the genome of the bacterium Mycoplasma codes, consisting of about 1.1 million base pairs. Having assembled the genome inside a yeast cell, they transplanted it into a cell from a closely related species, Mycoplasma capricolum. The success clears the way for developing and testing new variants of existing organisms.

“With this approach, we now have the ability to start with a DNA sequence and design organisms exactly like we want,” says Gibson. “We can get down to the very nucleotide level and make any changes we want to a genome.”

Gibson says that his team is now trying to make different types of synthetic cell, using different pairs of bacteria. The group also plans to use the approach to continue work on its project to create a ‘minimal’ cell that contains only the genes necessary for a cell’s most basic survival. “We finally have an assay for determining the functionality of a genome,” says Gibson. “So we want to start whittling away at this genome and try to determine the smallest number of genes needed to sustain life.”

Several companies are already seeking to take advantage of the new field, called synthetic biology, which combines chemistry, computer science, molecular biology, genetics and cell biology to breed industrial life forms that can secrete fuels, vaccines or other commercial and medicinal products. They edited that file, adding new code, and then sent that electronic data to a DNA sequencing company called Blue Heron Bio in Bothell, Wash., where it was transformed into hundreds of small pieces of chemical DNA, they reported.

To assemble the strips of DNA, the researchers said they took advantage of the natural capacities of yeast and other bacteria to meld genes and chromosomes in order to stitch those short sequences into ever-longer fragments until they had assembled the complete genome, as the entire set of an organism’s genetic instructions is called. They transplanted that master set of genes into an emptied cell, where it converted the cell into a different species.

Dark Chocolates May Protect Brain After Stroke

Researchers at John; Hopkins have discovered that a compound in dark chocolate may protect the brain idler a stroke by in­creasing cellular signals already known to shield nerve cells from damage.

Ninety minutes after feeding mice a single modest dose of epicatechin (a compound found naturally in dark chocolate), the scientists induced an ischemic stroke by essen­tially cutting  blood supply to the animal brains. They found that the animal suffered significantly less brain damage than the ones that had not been given the compound.

While   most   treatments against stroke in humans have to be given within a 2-3 hours time Window to be effective; epicatechin appeared to limit further neuronal, damage when given to mice 3.5 hours after a stroke. Given six hours after a stroke, however, the compound offered no protection to brain cells.

Sylvain Dore, Ph.D., associate professor of anesthesiology and critical care medicine and: pharmacology and molecular sciences at the Johns Hopkins University School of Medi­cine, says his study suggests that epicatechin stimulates two previously well-established pathways known to shield nerve cells in the brain from damage. When the stroke hits, the brain is ready to protect itself because these pathways – Nrf2 and home oxygenase 1 — are activated.

The study now ap­pears online in the Journal of Cerebral Blood Flow and

metabolism.

Eventually,   Dore says, he hopes his re­search into these pathways could lead to insights into limiting acute stroke damage and possibly protecting against chronic neu­rological degenerative conditions, such as Alzheimer’s disease and other age-related cognitive disorders.
